Interested in serving as the vital link between medical marijuana patients and proprietors? Permitted via certification from the Arizona Department of Health Services, caregivers are crucial because they provide medical marijuana to eligible patients who may be physically unable access their nearest dispensary because of the severity of their illness or proximity to their zone’s dispensary.
In order to receive your Medical Marijuana identification card you must submit fingerprints to the Arizona Department of Health Services (ADHS). Any previous fingerprint clearance will be ineligible to obtain your caregiver identification card because these clearances do not check a list of “excluded felony offenses”, which caregivers are checked for by the ADHS. Caregivers are required to submit two sets of fingerprints to the ADHS in an acceptable format. When the ADHS has concluded the process they will proceed to destroy all records of them.
Once you have filled out your online application, you will be directed to print your application. With the printed application, the Fingerprint Submission Information sheet, and the copies of your finger prints, you will then be able to send all the documents to the ADHS at the following address:
